A student must complete the Request for Permit and Statement of Intent to Employ a Minor (B1-1). These forms are available in the Career Center, or at www.dir.ca.gov/dlse/DLSE-CL.htm
Completed Request for Permit and Statement of Intent to Employ a Minor (B1-1), must be submitted to the Career Center for processing.

Students with two or more fails on their last report card will not be eligible to receive a work permit. |
